2010 Pledge Form & Report Form

All state employees will receive a pledge form from their local SECC Coordinator. A printable PDF version is also available. A Word document of the agencies and federations that are eligible for donations is also available.

If you need additional information regarding the pledge form, please call our offices at (919) 821-2886. When sending pledge information to the SECC offices, coordinators need to include a report form. A report form and special event form was included in the materials mailed to your offices, however you can download an additional report form or special event form you have multiple mailings.
